Job Title – Bilingual Central Navigator
Language Requirement: Fluent in English and Spanish
Position Status – Full-Time (40 hours per week -Salary – Non- Exempt Status)
Location – Position will serve Dodge & Washington counties and will be based in Fremont. It is preferred that the applicant reside in Dodge or Washington county
Salary Range- $41,000+ based on experience

 General Summary:                                                                                                                  
 The Bilingual Central Navigator serves as a critical point of contact for community members seeking access to services and resources as well as serve as a liaison with our local public schools.  This role involves providing culturally and linguistically appropriate support, making referrals, and coordinating care or services across programs. The ideal candidate is compassionate, resourceful, organized, and fluent in both English and Spanish.
 
The mission of the Fremont Family Coalition is to create community partnerships that empower individuals and families to improve their quality of life.
 
Essential Duties and Responsibilities                                                                                      
   
 Work directly with families in the Community Response prevention system
  • Accepts referrals calls and forms from individuals and partnering agencies
  • Meets with providers/families to assure all intake information is received to assess their needs, goals, and eligibility for other services and supports
  • Gathers information from incoming referral agencies and families and assesses the needs of the family to help identify and refer to appropriate programs, services, or supports the individual would be eligible to participate in
  • Utilizes a family centered approach to identify current supports and services to increase protective factors
  • May be first point of contact with a family/individual (self-referral when family is not connected by another care management/referral agency) and will provide fast turn-around and knowledge on community resources and supports 
Maintain positive relationships with partner agencies
  • Assists in conducting outreach activities to inform agencies and families in the area of the initiative and available services resources
  • Maintains regular communication of service eligibility and plan with other agencies and services
  • Attends trainings, presentations, and meetings regarding protective factors and family centered practices

Other knowledge, skills, and abilities

  • Knowledgeable and sensitive to issues about trauma and its effects and supports trauma informed changes in service delivery
  • Develop and maintain a working knowledge of services available to children and families in our community
  • Assist in identifying community needs, service gaps, service barriers, implementation strategies, and inter-agency partnerships
  • Ability to maintain strict confidentiality of sensitive information in accordance with HIPAA regulations
  • Ability to work both as an integrated team member and independently with minimum supervision
  • Enthusiasm and commitment in the value of investing in individuals
Qualifications: Training and Education                                                                                   
  • Bachelor’s degree in social work or related field preferred
 Qualifications: Experience                                                                                                      
  • Two years of relevant experience preferred
 Qualifications: Licensure/Certification                                                                                    
  • Must have a valid Nebraska driver’s license and provide proof of personal driver’s insurance and the ability to utilize own transportation for work purposes
