Care Provider
JOB SUMMARY:
The responsibility of the Care Provideris to provide personal care and homemaker services to the home care client and family as detailed in the plan of care, under the direction of the RN. They will work as a member of the home care team in providing direct care within the guidelines of the care plan.

Physical Requirements:
This position requires moderate physical effort intermittently during the shift including kneeling, squatting, reaching, twisting, lifting, walking, exposure to temperature and humidity changes and reasonable and safe assistance in lifting and/or transferring of clients. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
Mental Requirements:
Must be able to work independently to follow a plan of care and complete tasks in a timely manner. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and stay calm in a stressful situation. Must be able to utilize conflict resolution practices and de-escalation techniques. Must be able to manage time effectively and find tasks to do in down time to improve the quality life of the client(s).
Working Conditions:
Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions, and hazardous materials. Proper PPE is available and provided by the company. It is the responsibility of the caregiver to come to the office to pick up this PPE.
Transportation:
Must have a current valid driver’s license, valid auto liability insurance and reliable transportation.
Essential Functions:
1.Under the direction and ongoing supervision of the CEO/Administrator, provides essential personal care assistance to the client:
•The prompt, consistent, and reliable arrival of the caregiver to scheduled shifts on time.
•Bathing, care of mouth, skincare and hair care.
•Bathroom or in using bedpan or commode.
•Bed transfers as well as ambulation activities as ordered under the care plan.
•Mechanical lift transfers including but not limited to hoyer lift and sit/stand lifts.
•Range of motion exercises which the Aide has been taught by appropriate professional personnel.
•Performing such incidental household services as are essential to the client’s well- being at home.
•Preparing meals.
•Takes a client’s temperature, pulse, respirations and blood pressure if assigned by a registered nurse.
•Assists with shopping.
•Safe and Reliable transportation of clients.
•Feed Assistance as needed and as ordered in care plan.
•Catheter and Ostomy Care (if applicable and if trained)
•Medication Reminders
•Hospice Support and End of Life Caregiving
•Pet care (letting outside if needed, litter box cleaning, feeding/ watering pets, bathing and exercising pets as needed).
•Basic snow removal for caregiver and client entry and exit from the home as needed.
2.Provides emotional support and compassionate care to the client and family unit.
•Demonstrates listening skills.
•Offers encouragement when appropriate.
•Plans and participates in engaging activities such as outings, games, puzzles, etc.
•Practices patience in all interactions; voice and manner.
3.Maintains required competency and skills.
•Participates in annual performance review
4.Reports any changes in the client’s condition or family situation immediately by calling True North Home Care 402-981-8593.
5.Due to the requirements for EVV (electronic visit verification), specific Technology Requirements include:
•The use of the AxisCare app via mobile phone to clock in, clock out, view care plan, document visit notes, request shifts, decline shift requests, message with supervisors for non-urgent needs, and view schedule/calendar.
•The use of a mobile phone to respond to supervisor calls and texts during business hours or during working hours in a prompt manner.
6.Adheres to all policies and procedure guidelines as well as appropriate OSHA guidelines.
7.Attends required trainings and meetings. (Considerations will be made)
8.Maintains current certification as required by the State (if applicable).
9.Maintains current knowledge and practice of infectious disease protocols.
10.Any other reasonable tasks or responsibilities as delegated by supervisor.
11.Such other duties as may from time to time be assigned by CEO/Administrator or supervisor.
